In order to play in any Queer Soccer game, including indoor and outdoor, you are required to fill out the Queer Soccer League Liability Waiver & Emergency Contact Form.

Even if you purchase a ticket, you will not be permitted to play until this form is filled out.

The purpose of this form is to protect QSL in case of medical emergency and to put a contact name on file in case something happens to you on the field.
